The most common alteration of plasma lipoproteins in non-insulin-dependent diabetes (NIDDM) is an elevation in triglyceride-rich lipoproteins (TRL). TRL consist of very low density lipoproteins (VLDL) derived from the liver and chylomicron and chylomicron remnants from the intestine.<BR>We examined the effect of glycemic control on the metabolism of chylomicron remnants using apolipoprotein (apo) B-48 as a marker for intestinal lipoproteins.<BR>Apo B isoproteins in TRL (d< 1.006 g/ml) were separated by SDS-polyacrylamide gel electrophoresis, and the apo B-48 ratio (precent of apo B-48 in total apo B) was calculated.<BR>Forty diabetic patients showed significantly (p<0.001) higher ratios of apo B-48 (3.9±1.4: mean±SD) than age-matched controls (2.3±1.0). HbAi levels were positively correlated with apo B-48 ratios (r=0.42, p<0.01). Another 27 diabetic patients with poor glycemic control and normal lipid levels were treated for 4 weeks. HbAi levels decreased from 12.3±2.6 to 10.7±1.9%. Most of the patients showed reduction in apo B-48 ratios. The decrease in apo B-48 ratios was statistically significant (from 4.4±1.8 to 3.2±1.3%, p<0.001).<BR>The data presented indicated that the diabetic patients had increased plasma levels of apo B-48 containing lipoproteins which were supposed to be improved by glycemic control. The increased plasma levels of intestinal lipoproteins observed in poorly controlled diabetic patients may contribute partly to the increased risk of atherosclerosis in these patients.
