A comparison between gel tests (GT) and our routine tube tests (RTT) for ABO/Rh (D) typing, screening and identification of antibodies, compatibility tests by indirect antigloburin tests (IAT) and enzyme method, direct antiglobulin test (DAT) were examined. Blood samples from 376 patients in Tokyo Women's Medical College Hospital were tested.<br>There were no discrepancies in typing and DAT between the results obtained by both methods. In GT and RTT for antibody screening, 92.7% consistent with IAT in 55 samples, 70.7% consistent with enzyme method in 58 samples were found. In these for compatibility tests, 94.9% agreement by IAT in 78 samples, 100% agreement by enzyme method in 74 samples were found. In these for identification of antibodies, no discrepancies were found in all clinically significant antibodies (i. e. anti-Jka, anti-Fyb, anti-Dia, anti-S). Most of anti-Rh antibodies except IAT reactive antibodies were more sensitive in GT. Some cold reactive antibodies were not detected in enzyme method by GT, nor did they react in IAT. These were no clinically significant antibodies. In compatibility tests, GT was more sensitive to detect anti Jka and anti-c but was marginally reactive to find anti-Fyb.<br>Though the gel tests require a little improvement, they have equal or more sensitivity and specificity in detecting hemagglutination reactions in comparing with routine tube tests. This test offers the further advantages that reactions are stable, reproducible, more objective and easily interpreted. Moreover, this will be a valuable tool in small laboratories, for less-experienced technicians and in emergency at night or holidays.
