Abstract

Endoscopic examination of the upper GI tract were underwent on 55 out of 211 patients mainly with CVD admitted for rehabilitation from October 93 to October 94 to detect 27 pathological changes including four malignancies; two advanced and two early gastric cancers, which were resected by either surgical or endoscopic procedures. In all the cases, physical function was recovered to preoperative level by means of early postoperative exercise.
As benign diseases, 15 gastric ulcers, four gastric polyps and two esophageal varices were noted. Assumptive etiology of gastric ulcers in patients in non-acute phase of CVD was some psychological stress due to disabled levels of hemiplegia, or communication problems due to aphasia. Adverse effects of drugs for enhancing cerebral circulations, hypertension or diabetes, or non steroidal antiinflammatory drugs may also be a cause of gastric ulcer. In patients with CVD, unbalanced visceral innervation may mask symptoms, or their own discomfort may not be expressed due to aphasia.
We emphasize an importance of endoscopic examination for screening diseases of upper GI tracts in patients with CVD even without complaints.
