Continuous and pulsed wave Doppler echocardiography were performed in 65 patients with the aortic prosthetic valves to test the usefulness of the parameters by the continuity equation, i. e. effective orifice area (EAO), Doppler velocity index (DVI), aortic stenotic ratio (ASR), comparing them with the maxPG by the Bernoulli equation. The results were 1) DVI and ASR were independent of the valve sizes in contrast to maxPG and EOA, which were significantly influenced by them. 2) EOA, DVI, and ASR were correlated inversely with the maxPG. 3) Reduced opening angles of the prosthetic valves were observed in the patients who showed the low DVI and ASR despite of the normal maxPG, since the blood flow through the prosthetic valve was decreased by other factors, such as mitral prosthetic valve, mitral stenosis, mitral regurgitation, and low stroke volume, etc. 4) DVI 0.25 and ASR 0.27 are thought to suggest the dysfunction of the prosthetic valve in the aortic position. We concluded that DVI and ASR by the continuity equation are easier and more useful for the functional assessment of the aortic prosthetic valve than the maxPG which is more affected by various conditions.
