This report provides retrospective follow-up data on 219 patients who received Hancock porcine bioprosthetic valve between February, 1975 and October, 1981. Cumulative follow-up was 1327 patient years (mean 6.0 years). There were 35 late deaths (late mortality rate 2.6%/patient-year). Primary tissue failure (PTF) occured in 19 patients. Reoperation was performed at a mean interval of 7.1 years after the first operation (mortality 11.1%). At 8, 9, 10, 11 years after operation, actuarial rate of freedom from PTF was 90%, 81%, 76%, 55%, respectively. Thus PTF rate increased significantly after 9 years postoperatively. The incidence of PTF in patients under 50 years of age was higher compared with those over 50 years (1.6%>0.9%/patient year). PTF was caused by fibrocalcific obstruction in two, tissue over ingrowth in two and carp stiffness with calcium deposition in two. The remainder showed severe incompetence because of cusp tear or perforation. Reoperation for PTF may be a major concern, although mortality of elective cases is low. Specific factors leading to PTF require further investigation.
