Abstract

Cell-mediated immunity to human red cell membrane of O-type was investigated by Sφborg's leukocyte migration test, in six patients with idiopathic acquired immune hemolytic disease, one SLE patient associated with symptomatic acquired immune hemolytic disease, one patient with hepato-thyroiditis syndrome (Fukase, Ito) associated with Coombs' test-negative hemolysis, one patient with Hodgkin's disease associated with Coombs' test-negative hemolysis, and one patient with paroxysmal nocturnal hemoglobinuria. 1) LMT with red cell membrane was positive in one and weakly positive in three out of seven patients with Coombs' test-positive acquired immune hemolytic disease. One patient who had shown weakly positive LMT, became nagative with the LMT under the immunosuppressive treatment. 2) One patient with hepato-thyroiditis syndrome associated with Coombs' test-negative hemolysis showed weakly positive LMT with red cell membrane. However, the LMT in a patient with Hodgkin's disease associated with Coombs' test-negative hemolysis was negative. A patient with paroxysmal nocturnal hemoglobinuria showed negative LMT with the same antigen. It was suggested that CMI as well as humoral immunity to red cell membrane would be an important pathogenetic factor in some idiopathic and symptomatic acquired immune hemolytic diseases.
