The pits and fissures of occlusal surface of molar teeth are the most susceptible site for dental caries in a mouth. If the contour correction of occlusal surface artificially improved self cleansing and diminished the formation of bacterial plaque adjacent to the entrance of pits and fissures, dental caries might be largely prevented.<BR>In order to study the effect of occlusal contour correction, one hundred thirty-nine pairs of sound first molars of both sides in same jaw were selected from school children aged seven to eight in a farm village of Iwate Prefecture. The occlusal plapue of selected teeth was disclosed with erythrosin solution and was scored 0 to 3 following by the criteria of Occlusal Plaque Index. After then, the occlusal surfaces of one side teeth enrolled to experimental group were slightly cut away as showing Fig 1 with the use of identical abrasive points, and the others enrolled to control group were not operated. The occlusal surfaces were examined twelve monthly for forty-eight months, and following finding were obtained. The caries reduction by occlusal contour correction was constantly around fifty percent in every examinations and the result was statistically highly significant. While the reduction of occlusal plaque after the operation was obvious in experimental group, similar improvement was also observed in control group. The differences of the plaque estimates between experimental aud control groups were not distinct, except that one of forty eight months after was statistically significant at probability of 0.05.<BR>From the above mentioned results, it is concluded that the occlusal contour correction is useful for caries prevention of occlusal surfaces of molar teeth but the reducing effect of occlusal plaque by this method is not so distinctly.
