Since 1971, more than one thousand patients with habitual angina, chronic tonsillitis and tonsillogeneous focal infection have been treated by splitting the tonsils with an electrically heated cautery.This procedure is called thermosurgery by the present authors.<br>The postoperative follow-up for three years in 120 patients with habitual angina is as follows:no recurrence;81.7%, improved;11.7%, recurrence;6.7%, worse than before;0%.Thus the results of thermosurgery were almost the same as those of tonsillectomy.Most pro-minent histological findings of the tonsils treated by thermosurgery were marked enlargement of lacunae, reduction of size and number of germinal centers and lack of scar formation.The enlarged lacunae seemed to prevent retention of inflammatory products and supress the recurrence of tonsillitis.The treated tonsils preserve their proper tissue and probably function which will be immunologically important.The instrument and technique are simple, easy and safe.If satisfactory improvement is not obtained by thermosurgery, tonsillectomy can be performed afterwards.We recommend this method first before tonsillectomy is attempted.
