Type 4 thyroplasty (TP4) is a pitch elevation surgery that increases the tension of the vocal folds and elevates the speaking fundamental frequency (SFF). We combined TP4 with arytenoid adduction and type 1 thyroplasty as pitch elevation surgery. We carried out TP4 in two patients with menopausal voice disorder, and report the results herein.<br>Case 1 : female, 72 years old. This patient had undergone four years of hormone replacement therapy (HRT) for a menopausal disorder. Her menopausal symptoms had improved, but the pitch of her voice remained low. When she came to our hospital for pitch elevation surgery, her SFF was 186.1 Hz and her lowest pitch was 157.8 Hz.<br>Case 2 : female, 50 years old. The pitch of this patient's voice had gradually lowered over two years, making her feel uncomfortable at her workplace. HRT had been started around the same time following a diagnosis of menopausal disorder. Improvement in her menopausal symptoms had not been accompanied by improvement in the low pitch of her voice. When she came to our hospital, her SFF was 178.3 Hz and her lowest pitch was 130.8 Hz.<br>In both cases, TP4 was performed under local anesthesia, and with voice monitoring the tension of the vocal folds was regulated to achieve an appropriate pitch. The postoperative SFF in Cases 1 and 2 rose to 323.7 Hz and 251.1 Hz, respectively. We believe that TP4 is a useful method in the treatment of menopausal voice disorder.
