Clinical Evaluation of Treatments of T1/T2 Hypopharyngeal Squamous Cell Carcinomas

Abstract

We analyzed the clinical features of 99 patients with T1 or T2 squamous cell carcinoma of the hypopharynx who received radical treatment at the Department of Otolaryngology-Head and Neck Surgery, Kurume University Hospital, between 1989 and 2003.<br>The patients consisted of 85 males and 14 females (mean age : 64 years) ; T classification was 17 T1 cases and 82 T2 cases. N classification of the patients was 47 N0 cases, 18 N1 cases, 30 N2 cases, and 4 N3 cases. Nine were stage I, 38 were stage II, 18 were stage III, and 34 were stage IV.<br>In 41 patients, the primary disease was treated by laser debulking surgery prior to radiotherapy (LDSR). Twenty-five cases were treated by pharyngo-laryngo-esophagectomy and 14 cases were treated by partial pharyngectomy. In the partial pharyngectomy group, preservation of the larynx was possible in 10 cases, but laryngectomy was perfomed in 4 cases. Nine cases were treated by radiation only. Concurrent chemoradiation was carried out in 7 cases. Laser resection alone was performed in 3 cases. As a result, laryngectomy was performed in a total of 28 cases and 70 (71.4%) cases were treated while preserving the larynx.<br>The disease-specific 5-year survival rate was 76.6% (T1 : 87.4%, T2 : 74.8%). The 5-year local control rate was 86.9% (T1 : 70.0%, T2 : 89.5%). Local recurrence was recognized in 10 cases. In 7 cases, recurrence occurred in the form of neck lymph node metastasis. Distant metastasis was recognized in 9 cases. Twenty patients died of the primary disease (locoregional death : 1, lymph node death : 6, distant metastasis death : 9, complicant death : 3, surgery death : 1). This retrospective review indicates the necessity of supportive chemotherapy for T1/T2 hypopharyngeal carcinoma following radical treatment.
