There is increasing evidence that chemotactic factors (CF) play an important role in granulomatous inflammation in patients with sarcoidosis. It has been suggested that lymphocyte-derived CF are necessary for initiation and maintenance of granuloma formation. One of us has established a culture method to produce monolayers of epithelioid cells from sarcoid granulomas. We used this method to test whether cultured lymphocytes and epithelioid cells from sarcoid granulomas produced CF for neutrophils and mononuclear cells.<br>Cells were obtained by scalene lymph node biopsy from 15 non-treated patients with active sarcoidosis. The tissue was minced and cell suspensions obtained by passage through a cytosieve mesh screen. Cells were cultured at 37°C in 5% CO2 and supernatants were collected 1 week later. Monolayers of either epithelioid cells or floating lymphocytes were then cultured for an additional week to obtain supernatants. Chemotactic activity for either neutrophils or mononuclear cells was assessed using modified Boyden Chambers. Zymosan activated sera were used as positive controls and medium alone as a negative control. Each supernatant was placed on Sephacryl S-200 to estimate molecular weights.<br>The results indicate that i) CF for mononuclear cells and neutrophils from normal individuals were detected in supernatants from cultured sarcoid granulomas, ii) similar activity was detected in culture supernatants from both lymphocyte and epithelioid cell cultures. These factors were different by molecular sieving, iii) neutrophils from patients with sarcoidosis were less responsive to CF and did not respond well to CF derived from their own granulomas, whereas, mononuclear cells from patients with sarcoidosis responded well to CF.<br>In conclusion, CF are released from cultured sarcoid granulomas and they appear to preferentially attract blood-derived mononuclear cells.
