A 41-year-old male who had been suffering from Ph1-positive chronic myelogenous leukemia for 5 year was admitted to the Keio University Hospital on 16 August 1982 because of occasional nasal bleeding and easy fatigability. A peripheral blood examination on admission showed marked leukocytosis with a blast content of 10 percent and thrombocytopenia. Chromosomal aberrations, in addition to the high blast content, indicated that the patient was in the blastic phase.<br>The patient responded well to treatment with vincristine and prednisolone, but suddenly developed marked tachycardia and cyanosis while in a hematologically well-cotrolled condition and expired within a few hours. An autopsy revealed an enlarged pericardial sac containing a straw-colored effusion and a diffuse infiltration of immature leukemic cells into the patient's pericardium. Acute cardiac tamponade due to leukemic cell infiltration was confirmed as the cause of death.
