Description

Here, we report a case of erythema exsudativum multiforme (EEM)-type drug eruption. A 38 year-old patient was treated with intravenous drip infusion of clindamicine and oral intake of minocycline chloride. Together with fever up to 39°C and mild liver involvement, EEM-like lesions appeared on the lower extremities and subsequently expanded to the whole body. Upon hospitalization to our university clinic, clindamycine and minocycline was interrupted under the suspicion of drug eruption. Fosfomycin was then, administered. Oral administration of 30mg prednisolon resulted in rapid resolution of symptoms, including skin rashes, fever and liver dysfunction. The specific IgM and IgG antibody titers of human herpesvirus type 6 (HHV-6) were elevated. The patch testing revealed positive reactions with various concentrations of clindamycine (10%, 1%, 0.1%) but not with other drugs. Finally, she was diagnosed having a drug eruption (EEM-type) due to clindamycine.
