Abstract

Objective: We examined the results of smoking cessation therapy with nicotine patches or varenicline in Ningen Dock or health check-up subjects as well as factors influencing those results and changes in weight.
Methods: Our subjects were patients who underwent nicotine replacement therapy or varenicline therapy at the Seirei Preventive Health Care Center from February 2006 onwards. Ninety-six persons were prescribed nicotine patches (77 men,19 women, mean age ± SD: 50.9 ± 13.5 years) and 77 persons were prescribed varenicline (67 men, 10 women, mean age ± SD: 51.4 ± 11.3 years). We evaluated the success rates of smoking cessation for all subjects, by therapy and gender at the end of therapy (after 12 weeks) and 1 year later. We also examined associations between success in smoking cessation and various factors (sex, age, TDS, numbers of cigarettes smoked per day, years of smoking, Brinkmann index, expiration CO density in initial medical examination, period of therapy, age at start of smoking, confidence in smoking cessation, drinking, stress, smoking of family living together, underlying diseases, mental diseases). Change in weight versus success or failure in smoking cessation was also examined.
Results: In the nicotine patch therapy group, the numbers of subjects followed up at the end of therapy (12 weeks from its start) and at 1 year were 65 and 51 (96 originally), respectively. In the varenicline therapy group, the numbers followed up were 68 and 46 (77 originally), respectively. The smoking cessation success rate in the nicotine patch therapy group was 53.8% at 12 weeks and 35.0% at 1 year. The rates in the varenicline therapy group were 76.5% at 12 weeks later and 48.0% at 1 year later. The factors associated with success were male gender, longer treatment period and higher age at start of smoking. Body weights in persons with long-term success in smoking cessation (1 year) were increased at 12 weeks but had stabilized at 1 year. In those who only achieved short-term success, there was a similar change at 12 weeks but body weights had returned to the starting level at 1 year.
Conclusions: It seemed that the smoking cessation therapy in Ningen Dock and health check-up subjects contributed to primary prevention of smoking related diseases. The success rates for smoking cessation were similar to those achieved in previous studies. The long-term success rate for smoking cessation with varenicline was 48%.
