The participation rates in a population-based screening survey for stomach and colorectal cancer in Ibaraki Prefecture, Japan were 13 and 16 % of the local population, respectively, while the average participation rates in the nationwide survey were 12 and 18 %. It is said that the participation rate in the stomach cancer screening program is declining. However, this study conducted in Ibaraki Prefecture indicated that the overall participation rates in the stomach and colorectal cancer screening program for the local population, workplace, and medical checkup are not decreasing (the rates were 41 and 36 %, respectively). According to the nationwide survey of the stomach and colorectal cancer screening program, 60 % of the participants in the population-based screening were female and two-thirds of those were over 60 years old. On the other hand, in the workplace-based screening and the medical checkup based screening, 60 % of the participants were male and two-thirds of those were aged 40 to 59. Based on the guidelines of the Japanese Society of Gastroenterological Cancer Screening, the parameters used to analyze the effectiveness of the screening program, such as the recall rate for further examination, the percentage of individuals who underwent further examination, and the cancer detection rate, were officially reported only for the population-based screening. This study revealed that those parameters were not evaluated in more than half of the total participants of the various screening programs. In Ibaraki Prefecture, the overall participation rate in the stomach and colorectal cancer screening program was 40% of the local population. In the population-based screening, first-time participants consisted of 20 % of the local population. These results suggest that in order to increase the participation rate in the stomach and colorectal cancer screening program to more than 50 % of the local population, healthcare professionals should make greater efforts to deliver educational messages aimed at increasing public awareness of those cancers.
