Actual condition of findings on readings in stomach X-ray examination and their standardization: for improving reading accuracy

We investigated the results of readings, according to abnormal findings and committee members, from a mass screening for stomach cancer conducted in Shimane Prefecture (Fiscal 2010). Using new radiography methods, “deformation” was the most common overall finding (27.3%), and indirect findings were the primary abnormal findings. A strong positive correlation (r=0.716) was observed indicating an association between “deformation” and the recall rate. The positive predictive value for “deformation” was extremely low at 0.12%, which indicated the need for a more reliable reading method. In order to determine whether they were uniform for each patient, readings were individually conducted for five patients with depressed or flat-type early-stage stomach cancer (taken from Revised Version 1 of the New Stomach X-ray Guidelines1)) and findings were based on judgment reports by 34 reading physicians. The results showed a clear variation in findings within the same patients, indicating the need for unification of terminology, clarification of definitions, and standardization of X-ray reading procedures. Therefore, in order to clarify the basis by which reading physicians reach judgments associated with abnormal findings, we proposed a new reading procedure.
