<p>在宅酸素療法（HOT）の適応基準は，高度慢性呼吸不全例の対象患者において，動脈血酸素分圧が 55 Torr以下のもの，および 60 Torr 以下で睡眠時または運動負荷時に著しい低酸素血症をきたす者であって，医師が在宅酸素療法を必要であると認めた者とされる．しかしながら，わが国の全国調査によると，PaO2が 60 Torr以上でHOTが導入された，いわゆる“適応外症例”は1/3以上に及んでいる．このような対象におけるHOTの意義についてはまだまだ不明な点が多い．今後，疾患ごとのHOTの効果と適応基準について，その意義と限界を医療経済的な側面も踏まえ多方面から検討する必要があろう．</p>
