Description
By a trigger of an adverse event at a university hospital in 1999, the Ministry of Education and the Directors' Association of the Japanese National University Hospitals issued the statement in order to strengthen function of university hospitals in 2001. In the paper, establishment of Japanese National University Council for Clinical Quality Management (JNUCCQM) was proposed and the first general assembly of the Council was held at Tokyo in 2002. Department of Clinical Quality Management of Osaka University Hospital has been assigned for the secretariat and the director of the department, Prof. TAKEDA, Hiroshi has been the president of the council. JNUCCQM holds conventions twice a year and consists of three sectional meetings (medical managers, full-time clinical risk managers, administrators). Major purposes of the Council are to share the information related to patient safety, to develop core curricula for training clinical risk managers and medical staff, to develop guidelines for patient safety, and to propose health policies related to patient safety to the government, industrial associations, medical institutions and citizens. Through activities of JNUCCQM, national university hospitals have developed standardized patient safety to some extent. Among them, there are an institutional incident reporting system, an organizational mechanism for patient safety, and a guideline for press release of serious adverse events. After the year of 2004 when all the national university hospitals were transformed to an “agency”, JNUCCQM has been in the second phase because each agency has to deal with matters on medical malpractice resolution including puchase of a malpractice insurance and conflict management. As little knowledge has been accumulated and skills for clinical risk management are not matured. JNUCCQM will play an important role in Japan for not only patient safety but also clinical risk management.
