抄録

We have experienced a 29-year-old male patient with duodenal rupture due to blunt abdominal trauma. The patient was suspected of perforation of the gastro intestinal tract 24 hours after trauma. GI series with watersoluble medium revealed a traumatic duodenal rupture. Since he was a "Jehovah's witness" and denied to receive blood transfusion preoperatively, we accepted his proposal and operated on. On operation, it was found that the second part of the duodenal intestine was cut off and could not be repaired, and pancreatoduodenectomy was carried out. Operative bleeding amounted to about 300ml. The patient could be discharged on 90th day after operation without blood transfusion. <BR>Duodenal rupture due to blunt abdominal trauma is difficult to be diagnosed. In a case suspecting of an injury on the duodenal intestine, active apply of roentgenography with water-soluble medium is recommended. Moreover, if a patient is an adult "Jehovah's witness", clearly decleares a denial of blood transfusion, and gives a written consent assuring not be call a surgion to account, we can interpret that any legal probloms are not discussed even though the patient dies because he or she dose not receive the blood transfusion, or undergose it contrary to his or her will.
