An 82-year-old woman visited another hospital because of anal bleeding and was referred to the hospital with a suspicion of rectal tumor. Rectal examination revealed a soft tumor occupying half circle of the lumen. Barium enema study showed a tumor, about 5cm in diameter and having a cauliflower-like appearance, in the lower rectum. Endoscopic study showed the soft tumor with a granular surface 5cm proximal to the anal verge, which was easy bleeding. Biopsy of the tumor proved tubulo-villous adenoma. After admission the tumor repeatedly prolapsed through the anus. Transanal resection of the tumor was successfully performed under lumbar anesthesia. Histopathologically it was cancer in tubulo-villous adenoma that partially both structural and cellular atypism were severe. Villous tumor is relatively rare among colonic tumors, and its high probability of malignant chane creates a therapeutic problem. We clinicopathologically conclud that the local resection was proper in this case.
