We experienced a case of smooth muscle tumor of the small intestine in a shape of cystic tumor presenting with perforation.<br> A 47-year-old woman was seen at the hospital because of a right lower quadrant pain and fever. There were tenderness and rebound tenderness in the right lower abdomen. Abdominal ultrasonography revealed a 7cm tumor with enheterogenous internal echo in the right side of the uterus. Abdominal CT visualized a cystic tumor with enhanced margin and gas in the tumor. Torsion of the right ovarian cyst was suspected and an emergency operation was performed. At laparotomy, a cystic tumor was found in the upper jejunum with the ruputerd wall and there was accumulation of pus in the vicinity of the tumor. About 10cm of the small intestine including the tumor was excised. The excised specimen revealed a 70×63mm cystic tumor with the wall thickness of 2mm, necrosed tissue in the lumen and a communication with the small intestine.<br> Histopathologically, interlacing spindle cell was confirmed in the cyst wall, and SMA staining was positive. The definite diagnosis of smooth muscle tumor of the small intestine was made.
