Description
複雑心奇形根治手術には右室切開を伴うことが多く,時に冠状動脈走行異常のため重篤な合併症を起こす.しかし,幼児症例では選択的冠状動脈造影はむずかしく末梢動脈の処理に高度の外科技術を要し合併症を起こしやすい.今回,われわれは経静脈性,心室中隔欠損孔経由による選択的冠状動脈造影を考案し, pre-shapedカテーテルの試作および Fallot 四徴症を中心とする複雑心奇形への臨床応用を行い本法による冠状動脈造影に世界で初めて成功した.d型心血管構築では右冠状動脈造影は容易であり,右冠状動脈走行が術前に把握できれば安全に右室流出路切開が行え,本法は小児循環器医も実施できる方法でありその有用性は高く,今後広く臨床応用されるべき造影法と考えられる.
