Description

遠位型大動脈肺動脈中隔欠損を合併した新生児例に,我々が新しく開発した術式で根治手術を施行し救命したので報告する.<BR>症例は日齢20日の男児で,生後まもなく心雑音および呼吸不全を認めた.心エコー検査により遠位型大動脈肺動脈中隔欠損および心房中隔欠損と診断し,日齢26日に手術に成功した.右肺動脈は上行大動脈右後外側部より起始し,右肺動脈入口部と中隔欠損孔との間に距離があったため,手術には大動脈肺動脈中隔欠損孔が新しい右肺動脈入口部となるように直接吻合する新しい術式を用いた.この術式は,術後の吻合部狭窄の危険性が少なく,このタイプの遠位型大動脈肺動脈中隔欠損に有用であると思われる.
