Abstract

総肺静脈還流異常症は, チアノーゼ性先天性心疾患の中でも生後早期より呼吸不全,右心不全症状を呈し1歳未満の自然死亡率は80%以上と高く, まして20年以上の生存例は 1-4 % にすぎないといわれている. われわれは最近 Darling Ia型の2成人例に, Gersony-Malm法に準じ根治手術を施行し良好な結果を得た. 手術前後の心エコー図を検討し, 右心系容量負荷を特徴とする心房中隔欠損症の年長者例と同様に右室, 左室の Dimensionが術後早期にほぼ正常化していることをみとめた. 両症例は, 現在元気に日常生活をおくっており, 本症の予後,手術成績を左右する因子, 自然歴における生存条件について若干の文献的考察を加え報告した. 51歳の症例は, 本邦における手術例の最高年齢と考えられる.
