- 臨床 WPW症候群における副伝導路(Kent束)の不応期測定について

Description
Kent型WPW症候群3例について,副伝導路不応期の測定を試みた.3例共従来のRapid pacingによってはwide QRS のnormalizationはまったくみられず,ERPAPの測定は不可能でExtrastimulus法で以下のごとき種々の条件でのERPAPの測定を行った.Basic cyclelengthによる変化,およびOuabain ,Procainamide投与前後におけるERPAPの変化を検討した.ERPAPはBCLの短縮に伴い,短縮した.3例共,Ouabain投与後,ERPAPはBCLに関係なく減少を示した.その後,第1例,第3例にProcainamideの静注を行った所,ERPAPが延長した.3例共,心房細動によると思われる頻拍発作の既往を持ち,これに対するDigitalis使用はERPAPを短縮し,頻拍発作をさらに増悪する可能性があるので,注意を要する.
