38歳の女性,主訴は意識消失発作である.既往歴として2回の失神歴を有した.平成12年1月頃より,起立時の立ちくらみが頻発し始めた.同年12月,起立位で上司との会話中,立ちくらみが出現した.息をこらえ我慢していたところ意識消失発作が出現したため救急車で某病院に搬送され,当院紹介受診となった.理学的所見,非観血的検査に異常は認められず,臨床経過よりneura11y mediatedsyncopeが疑われたためhead-up tilt test,頸動脈洞マッサージを施行したが,失神発作は誘発されなかった.Valsalva試験は第II相,第IV相における異常反応が認められ,valsalva ratioは低下しており,自律神経系異常パターンが認められた.このことより,再度80度受動起立下で息こらえをしたところ,開始直後より急激な血圧の低下をきたし,約6秒後に臨床症状と同様の失神発作が誘発された.以後,日常生活での息こらえを必要とする動作の禁止を指導し,外来経過観察中である.息こらえにより失神発作が誘発される症例は,我々が検索した限り,成人例においては2報告にすぎず,まれな症例と考えられ報告した.
