症例は52歳,女性. Systemic Iupus erythematosus(SLE) , 高血圧, 陳旧性脳梗塞, 陳旧性心筋梗塞の診断通院中であった.2001年9月14日午後, 突然胸痛が出現し, ニトログリセリン舌下にて改善しないため, 救急車にて当科外来受診.心電図検査にて,急性心筋梗塞を疑われ入院となった.入院後胸部X線で縦隔の拡大を認め,心臓超音波検査で上行大動脈の拡大とflapが疑われたため,急性大動脈解離に合併した下壁梗塞と診断し,血圧コントロールによる保存的治療を開始した.9月27日に心臓カテーテル検査を施行.上行大動脈の解離のほかに,左前下行枝の完全閉塞と右冠動脈入口部の解離が確認されたため,10月22日に上行弓部大動脈人工血管置換術と冠動脈バイパス手術が施行された.<BR>SLE患者が血栓性疾患を起こしやすいことはよく知られているが,急性大動脈解離と急性心筋梗塞の合併例は少なく,今後注意すべき疾患として報告する.
