Abstract

A 72 year old man with otalgia, long standing chronic otitis media in his left ear, gait disturbance, vertigo, and vomiting was reported. Detailed examination, including CT and cerebral angiography, revealed that the vertigo resulted from the cerebellar infarction occuring simultaneously with acute mastoiditis and acute external otits. Serous otorrhea with a positive glucose detecting test in his left ear and severe contact pain limited to the second and third trigeminal area in his left ear were also detected in this patient. The former symptom was considered to be caused by acute external otitis (Psuedomonas aeruginosa infection) the latter one considered to be casued by cerebellar infarction. Antibiotic resistant acute inflammation and increase of thrombus formating trend had an important role in the occurence of the cerebellar infarction, in addition to arteriosclerotic changes of vertebulovasillar artery due to hypertension, heart disease and diabetes mellitus.
