Description

An 86-year old man had positional vertigo and nystagmus typical of benign paroxysmal positional vertigo (BPPV). He was treated 4 times by CRP (the canalith repositioning procedure) of Epley, and 4 times by the liberatory maneuver of Semont and by Brandt-Daroff physical therapy for 45 days, but his positional vertigo and nystagmus did not disappear. In the head hanging right ear down position or in the head hanging left ear down position during CRP, the head is usually at an angle of 45 degrees to the horizontal plane. Altered CRP, where this angle has changed to 60 degrees, was thus newly designed for this patient with intractable BPPV. As a result, he was cured after the altered CRP was performed only once.
