Is a safety guidewire necessary during flexible transurethral lithotripsy?

Abstract

Objective:When performing flexible transurethral lithotripsy(f-TUL), the placement of a safety guidewire(SGW)is generally recommended in the European Association of Urology guidelines 2013. <br>  We evaluated the necessity of placing an SGW. <br>  Patients and Methods:Between April 2012 and May 2013, we conducted 100 f-TUL procedures without and 24 with an SGW. <br>  We investigated the average size of stones, ureteral kinks, findings associated with calculi of the urinary tract, preoperative hydronephrosis, operative time, stone-free rate(SFR), and complications. <br>  Results:There were no significant differences in either the mean operative time(with SGW:44.5 minutes vs. without SGW:37.5 minutes)or stone-free rate(with SGW:91.7% vs. without SGW:95.0%). <br>  We tended to require placement SGW for patients with preoperative hydronephrosis, ureteral kinks or findings associated with calculi of the urinary tract. <br>  Conclusion:From this study, only a limited number of patients require SGW placement.
