A case of primary appendiceal adenocarcinoma with some interesting PET/CT and SILS findings

Abstract

A 62-year-old woman complained of right lower abdominal pain without any systemic or local signs. Laboratory tests demonstrated elevated serum CEA levels. Pelvic CT showed a small mass in the right pelvic cavity. PET/CT showed enhancement with FDG accumulation in the lesion. The initial preoperative suspicion was malignant epithelial tumor involving the vermiform appendix, but ovarian tumor was not excluded. Exploratory single incisional laparoscopic surgery (SILS) was performed. SILS confirmed that the mass originated from the appendix and infiltrated to the right adnexa uteri and distal mesoileum. Considering that this was most likely advanced cancer, the procedure was converted to open surgery. Frozen section pathological examination of the en bloc resected specimen demonstrated a primary appendiceal adenocarcinoma. Ileocolectomy was subsequently performed. In this case, we used PET/CT and SILS as diagnostic tools and found both of these to be helpful.
