<p>Objective: We report a case of developed MR imaging-enhancing brain lesions after endovascular therapy of an aneurysm.</p><p>Case Presentations: A 74-year-old man who underwent stent-assisted coiling of a 9 mm incidentally discovered basilar apex aneurysm. Approximately 3 weeks after his aneurysm coiling, he developed headache, double vision, and hallucination, and an MRI of his brain revealed bilateral white matter changes with ring enhancement. Corticosteroids were administered and the clinical findings and abnormal features on follow up MRI had improved. Catheter-coating embolization is documented in the neurointerventional literature. In recent report, intraprocedural foreign body emboli of one of the water-soluble polymers used for hydrophilic coating (PVP) have been associated with posttreatment delayed ipsilateral intraparenchymal hemorrhage and granuloma. In this case, imaging and clinical characteristics suggest a foreign body reaction such as granuloma. We suggest that the cause of these lesions might be shedding of hydrophilic coating from endovascular devices into the blood stream.</p><p>Conclusion: These findings are suggestive of a potential association between intraprocedural foreign body emboli.</p>
