Alcohol has been implicated in the development of a number of hematologic abnormalities, including various types of anemia, leukopenia, depressed leukocytes mobilization and thrombocytopenia, while in Japan few studies were reported.<br>Hematologic studies were carried out on one hundred and thirty-nine alcoholic patients.<br>1) Anemia<br>Sixty-seven cases (48.2%) were anemic as defined by R.B.C. below 3.5million/cmm. This incidence of anemia is lower than previous reports. Previous reports demonstrated that most anemic patients showed megaloblastic and/or sideroblastic marrow, but in my forty-five cases who had bone marrow examination seven cases (15.6%) of megaloblastic anemia and only two cases (4.4%) of sideroblastic anemia were found.<br>Bone marrow failure, disturbance of folate and pyridoxine metabolism have been con-sidered contributory factors to anemia. Bone marrow failure may be mainly due to 'vacu-olization' of erythroid precursor cells. Electron microscopic study demonstrated that 'vacu-oles' were lipid. Further, abnormalities of mitochondria and nuclear abnormalities of erythroid precursor cells suggesting disturbance of DNA synthesis were demonstrated. These changes rapidly disappeared after withdrawal of alcohol.<br>2) Leukopenia<br>Twenty-seven cases (19.4%) were leukopenic as defined by W.B.C. below 4, 000/cmm. 'Vacuoles' of myeloid precursor cells were found to be lipid on electron microscopic study. These changes also disappeared after withdrawal of alcohol.<br>3) Thrombocytopenia<br>Fifty cases among ninety-seven patients (51.5%) were thrombocytopenic as defined by platelet counts below 10, 000/cmm. Platelet counts returned to normal after withdrawal of alcohol.
