Description
Objective : This study was conducted to identify the reason of discontinuing process during esophageal training after laryngectomy in order to establish appropriate nursing support model. Patients and Methods : From The participants were 8 patients who were followed more than 3 years after laryngectomy and could not continue esophageal speech training. All participants were informed and consented before the entry. From January 2005 to August, the survey was conducted by semi-structured interviews, and analyzed by Modified Grounded Theory Approach (M-GTA). Results : The reasons of discontinuing process during esophageal speech training were classified into 3 categories and 11 concepts. The 3 categories were the following ; " conflicting feelings at the time of decision making of undergoing laryngectomy", "avoiding communication due to loss of voice", and "awareness of negative feelings that interfere participating in a patients' association and alsotheir intention to continue esophageal speech training". Failure of harmony with the patients' association may cause negative feeling toward esophageal speech with increased feeling of isolation, and it resulted in discontinuation of practice. Conclusion : In order to facilitate the continual esophageal speech training in patients after laryngectomy, it could be necessary to strengthen collaboration between the patients' association and nursing be very important to show concrete benefits from the training while setting an individual-based realizable goal.
