This report deals with some pathogenetic factors of Giardia induced malabsorption syndrome in patient with reduced secretory immunoglobulin A in duodenal aspirate. Control studies were carried out with asymptomatic giardiasis.<br>In the patient with reduced SIgA in duodenal aspirate, Giardia trophozoites and bacteria are present in numberless on the duodenal mucosa and dense filamentous coating of trophozoite covered to fuzzy coat of the microvilli. It is possible that these conditions can cause some disturbance of the surface absorption process of microvilli.<br>Competition by large number of Giardia and overgrowthed bacteria for dietary nutrients may be a factor of malabsorption.<br>Flagyl effectively eliminated the parasites in asymptomatic patient, but the patient with reduced SIgA in duodenal aspirate, Flagyl-therapy was not effective as a result of oneyear-follow-up.<br>In two healthy carriers of the cysts, after one year, giardia cysts can not detected in stool without treatment.
