Of all the bones in the maxillofacial area, the condylar process is one of the most susceptible to fracture. Correct anatomical reconstruction of the condylar process is an important prerequisite for re-establishing function. It is important to ensure that the operator is able to conduct anatomic reduction and rigid internal fixation under direct vision of the completely exposed fractured end. The retromandibular approach was first proposed by Hinds and Girotti in 1967. Compared with the submandibular approach, it is closer to the condylar process and offers better exposure to the fracture site. This report describes the retromandibular approach for reduction and fixation of mandibular condylar fractures. The study group included 6 patients who had undergone surgery for 8 condylar fractures. There was no permanent facial nerve damage, salivary fistula or ankylosis in any of the patients.<br>We suggest that the retromandibular approach should be the technique of choice for condylar fracture management, because it seems to be a safe and efficient method for reduction and internal fixation of condylar fractures.
