Improvement of oral manifestation and quality of life through ingestive treatment

Abstract

Ingestive treatment, which includes oral care, has been covered by medical insurance in Japan since 1994. Though many reports show that oral care is important, there have been no reports on the effects of ingestive treatment on the quality of life (QOL). Therefore, we assessed these effects for 5 to 79 (average 20.4) days on QOL: oral manifestation, ingestive examination, chest X-ray, blood cell counts, biochemistry, and tongue coating culture test in 32 patients with ingestion disorder. After ingestive treatment, the General Oral Health Assessment Index (GOHAI) and awareness level regarding oral hygiene improved and was followed by an improvement in subjective oral manifestation, objective oral conditions, and eating manner in the patients. Although the results of the modified water-swallowing test, continuous labial closure force, chest X-rays, blood cell counts, and biochemistry showed no change, momentary labial closure force was reinforced after ingestive treatment. Dysphagia and other side effects did not occur during ingestive treatment, and none complained of this treatment. We conclude that ingestive treatment improved not only maxillofacial-oral manifestations but also the QOL of patients with increasing self-efficacy, which is essential for the success of ingestive treatment. Oral feeding and patient satisfaction are important aspects for senior citizens to lead contented lives. Therefore, it is important to spread “ingestive treatment”, including oral care, training of the maxillofacial muscles for eating, speech, and phonation, to facilitate the maintenance of proper health in the oral cavity. For that purpose, construction in the medical system is necessary to establish ingestive treatment.
