Odontogenic keratocyst (OKC) shows characteristics of tumor. In the histological classification of WHO in 2005, parakeratinized OKC was classified as keratocystic odontogenic tumor (KCOT). However, there is no statement concerning orthokeratinized OKC in this version. To characterize the clinical features of OKC, 73 cases of OKC in the Department of Oral Surgery II, Osaka University Hospital from 1992 to 2006 were histologically grouped into parakeratinized type, orthokeratinized type, and combination type, and the proportion of each type, age distribution, favorite site, extent of lesion, association with impacted tooth, treatment, and recurrence ratio were examined. Results showed that 86% of 93 cysts were the parakeratinized type, 9% orthokeratinized, and 5% combination type. All cysts in the orthokeratinized type were associated with impacted third molars. There was no recurrence in the orthokeratinized type, while 12% of the parakeratinized type and 20% of the combination type recurred. These results indicate that the combination type belongs to KCOT and that the orthokeratinized type is different from KCOT and similar to dentigerous cyst.
