抄録

遊離脂肪酸はエネルギー基質であると同時にさまざまなシグナル分子の基質でもあり，インスリン作用，インスリン合成・分泌に影響を与える．肥満症にともなう過剰な遊離脂肪酸は，耐糖能を悪化させる．遊離脂肪酸によるインスリン作用の障害を（広義の）脂肪毒性，インスリン分泌能に及ぼす悪影響を膵β細胞脂肪毒性（狭義の脂肪毒性）と呼ぶ．最近，脂肪組織以外の臓器に蓄積する脂肪（異所性脂肪）の動態に注目が集まっており，各臓器で何らかの病的意義を有する可能性がある．<br>
