Abstract

膵癌に対する手術適応の判定には，MDCTによる診断が必須である．切除不能で化学療法の適応となる例や，画像にて質的診断が困難な場合にはEUS-FNAが有用である．MRCPは膵癌との関連が問題となっている，小嚢胞や分枝の拡張の拾い上げに有用で，また，IPMNの経過観察時，由来浸潤癌や併存癌の検出には拡散強調画像の併用が役立つ．膵癌と炎症性腫瘤の鑑別はERCP，EUSによる腫瘤像，胆管像，膵管像の各々の特徴を理解することが重要である．ERCPは微細な膵管の変化を描出可能で，さらに膵液細胞診から上皮内癌や小膵癌の発見に期待が寄せられる．また，造影剤を用いた超音波による新たな血流評価も試みられている．<br>
