Abstract

気管支喘息とCOPDの鑑別に有用なものとして，臨床症状・喫煙歴・肺機能検査（肺拡散能や肺過膨張所見）や喀痰検査・呼気中nitric oxide（NO）測定・胸部高分解能CTによるlow attenuation area（LAA）の存在などがあるがその鑑別は容易ではない．COPDの併存は，高齢者喘息の20～30％程度にみられ，喘息の難治化要因として重要である．治療の基本として禁煙が有効であり，薬物療法では吸入ステロイド薬と長時間作用性気管支拡張薬の併用が有用である．
