<p>73歳，男性．胃瘻造設術後だが，食道裂孔ヘルニアによる逆流性食道炎と出血性食道潰瘍を認め，percutaneous endoscopic gastro-jejunostomy（PEG-J）tubeを挿入した．翌日ショックとなり，腹部造影CTで絞扼性イレウスが疑われた．緊急手術の所見では，腸管膜根部でカテーテルを軸に捻転しており，広範囲小腸壊死を来たしていた．tube挿入後のフォローが必要と考えられた．</p>
