Abstract

The patient was a 24-year-old female who consulted our hospital with the chief complaint of right lower abdominal pain. Abdominal ultrasonography showed the target sign in the ileocecal region, and intussusception of the terminal ileum into the cecum was confirmed by abdominal contrast-enhanced CT. Since no clear tumorous lesion responsible for the intussusception was noted on colonoscopy, a diagnosis of idiopathic intussusception was made. Endoscopic reduction of the intussusception proved impossible. As there was no improvement of the condition on the plain abdominal CT taken on the following day, laparoscopy-assisted reduction was performed. Since the intussuscepted terminal ileum was hard in consistency and felt like a tumor, ileocecal resection was performed. Histopathological examination revealed no organic lesion, and the final diagnosis was idiopathic intussusception.

Adult idiopathic intussusception is rare and often difficult to diagnose preoperatively. In the case presented here, we diagnosed idiopathic intussusception preoperatively, and reduced and resected the lesion by laparoscopic surgery. The case is reported with a review of the literature.
