Description
<p>甲状腺低分化癌は高分化癌（乳頭癌ないし濾胞癌）と未分化癌との中間的な形態像および生物学的態度を示す濾胞上皮細胞由来の悪性上皮性腫瘍と定義される。病理形態学的には，低分化癌細胞は濾胞構造やコロイド産生に乏しく，充実性，索状，島状といった特徴的な増殖パターン（STIパターン）を示す。WHO分類の13年ぶりの改訂では，病理組織学診断基準，病因，発生機序，遺伝子変異，臨床的特徴，予後因子など内容が大幅に改定および追加された。これにより，低分化癌の病理診断上の混乱が解消されることが期待される。しかし，不完全な記載や対応も幾つか残されている。本稿では低分化癌の WHO分類第４版の改訂点，問題点を中心に解説する。</p>
