Abstract

<p>脂肪は重要なエネルギー基質であると同時に，生体の膜構造の維持に不可欠な栄養素であり，周術期には欠かせないものである。近年，院内のNST活動の普及などによる栄養療法への意識の高まりもあり，脂肪乳剤投与の重要性は認識されるようになってきた。しかし，本邦での脂肪乳剤の併用率はいまだ低いのが現状である。脂肪乳剤投与が敬遠されてきた要因は，1）本邦で使用される脂肪乳剤がn-6系脂肪酸を主成分とする大豆油由来であり，術後の炎症反応の悪化や，免疫能の低下を引き起こす可能性があること，2）脂肪乳剤投与下では微生物が増殖しやすいこと，など脂肪乳剤投与が特に周術期には発生して欲しくないリスクをはらんでいることであろう。これらの問題点に対して，適切な投与速度を遵守する，輸液ラインを24時間以内に交換するなどの対応をすることで，脂肪乳剤は安全に投与できる。</p>
