Abstract

The goal of treatment for rhinosinusitis is to secure both ventilation and excretion pathways from the paranasal sinus to the nasal cavity and to recover functional ciliary movement. Treatment guidelines for rhinosinusitis in Japan recommend irrigation therapy using a catheter. This paranasal sinus irrigation therapy using the YAMIK catheter began in 1991 in Japan. Despite a certain complexity of the procedure, this method has constituted a revolutionary approach to reduce inflammation of the maxillary sinus, as well as the ethmoid, frontal, and sphenoid sinuses. However, availability of the YAMIK catheter (fabricated with latex) became limited in Japan in 2005. As an alternative, the ENT-DIB sinusitis therapeutic catheter (fabricated with silicon) was developed in Japan and has been in clinical use since 2014. Use of this catheter should exhibit a similar treatment effect to that of the YAMIK catheter. In this study, we performed ENT-DIB catheter irrigation therapy for 24 patients (5 men, 19 women) with sinusitis. Visual analogue scale measurements were recorded for sneezing, nasal discharge, nasal obstruction, pain, and olfactory disorder before and after irrigation. After treatment, there was a significant improvement in all symptoms of acute sinusitis, and in all symptoms of chronic sinusitis except sneezing and pain, with no complications such as nasal bleeding or otitis media. These outcomes suggest that acute rhinosinusitis is a good indication for ENT-DIB catheter irrigation therapy, and this approach or another non-surgical therapy may also become a proactive choice for some cases of chronic rhinosinusitis. This method does involve a complex maneuver, but it is regarded as one of the primary procedures to be learned for an otorhinolaryngology specialist.
