Abstract

A 49-year-old woman with a suspected gastric submucosal tumor on a screening upper gastrointestinal series was referred for further evaluation. Upper gastrointestinal endoscopy showed extrinsic compression of the wall of the greater curvature of the upper gastric body. Contrast computed tomography during the equilibrium phase showed a 6-cm mass in the spleen with faint enhancement. Magnetic resonance imaging showed a mass with regular smooth margins that was isointense with the spleen on T1-weighted imaging and hypointense on T2-weighted imaging. Serum levels of soluble IL-2 receptor, a tumor marker, were within normal limits.<BR>Since preoperative diagnosis was difficult, a laparoscopic splenectomy was performed for a definitive diagnosis. Immunohistochemistry for CD8, CD31, and CD34 confirmed a diagnosis of sclerosing angiomatoid nodular transformation (SANT).<BR>SANT is a benign vascular tumor of the spleen that was first reported in 2004. Few cases have been reported in Japan. This case is reported along with a discussion of the relevant literature.
