A 39-year-old male assembly worker who had had spasmordic torticollis for a year and had not been improved by desensitization, autogenic training and hypnosis for 6 months, was remarkably improved by behavior therapy with EMG biofeedback for 4 months. The electromyographic activity was more larger in the left sternocleidomastoid than in the right and the high-voltage spike waves (400-500μV) were specifically recorded when the patient's head turned to the direction of the right spasm. In order to reduce these potentials to the normal level, the feedback stimulus consisted of light, tone and digital counter were presented contingently upon the occurrence of torticollis. The patient was instructed with trying to avoid these feedback stimulus as hard as possible. This type of feedback training was conducted continuously every day under one each of the following 5 superimposed conditions. (1) sitting with lying back (150°), relaxation (2) sitting with lying back (130°), relaxation (3) sitting with lying back (90°), relaxation (4) standing, conflict situation with color naming (5) sitting, hand-eye coordination with pursuitrating These postures and tasks were arranged from a easy condition to a difficult one. After these feedback training for 113 days, this highly motivated patient could go back to his previous real life. From the view point of clinical application of biofeedback based upon the present case, the following points are suggested. (1) Some combination of a desensitization procedure, shaping method and biofeedback could be used to facilitate adaptation to real life situations. (2) Feedback training could be used as a means of possibly building up resistance to and reducing abnormal physiological responses to stress in general, and possibly to specific stressful events.
