Abstract

<p>[Purpose] We report the effect of a combination therapy of low frequency repetitive transcranial magnetic stimulation (rTMS) to the primary motor area of the lower limb on the nonlesional hemisphere and intensive physical therapy (PT) on a stroke patient with lower limb hemiparesis. [Participants and Methods] The subject was a 70-year-old male with left hemiplegia 14 months after stroke onset. The primary motor area which presents ankle dorsiflexion in the nonlesional hemisphere was identified using functional MRI prior to rTMS application. One treatment session consisted of 20-minutes 1 Hz low frequency rTMS and 60 minutes PT. Two sessions were conducted per day. A total of 21 sessions were performed during a 15-day hospital stay. [Results] Compared to hospitalization, at discharge, improvements were seen in MAS of the left soleus muscle, FRT, BBS, cane walking time, TUG, and FIM. [Conclusion] At one month after discharge, the patient was able to walk without a cane. It can be said that the combination therapy was an effective treatment method for this patient. </p>
