Description

We describe a 70-year-old male patient with a postoperative refractory enterocutaneous fistula in an open, postoperative abdominal wound that was treated by cooperating ward nurses and outpatient nurses certified in wound, ostomy and continence nursing. Management was extremely challenging due to issues with pouching, exacerbated skin damage surrounding the wound, and frequent changes of wound dressing. The patient endured physical and emotional distress due to the extreme pain caused by the skin damage. Therefore, we drafted and implemented a nursing plan based on assessment of the patient's verbal descriptions and behavior. This plan relieved the emotional distress of the patient and motivated him to continue treatment. Integrated care is important when fistula management is complicated, and thus information should be shared among caregivers through the use of procedure manuals containing illustrative graphical information.
